Maternity Care Service Contract
1) Service Information and Limits
- This contract is effective from the day client are discharged.
- If the client is discharged later than the expected date of discharge, the client is responsible for the payment of the first day or one business day.
- The caregiver from NJ Sanhoo provides service of caring for the newborn baby and assisting the new mother in postpartum recovery. The service may extend to the husband but other members of the family are not included. (Additional service charges will be applied for other family members)
- The caregiver is responsible for cleaning (mother’s room and bathroom, newborn baby’s room and bathroom and kitchen only), laundry (mother and baby’s clothes if the laundry machine is available), preparing meals (baby, mother and husband; additional charges applied for other family members), and other services related to maternity care only. If additional services are needed, client must make agreement with the caregiver and NJ Sanhoo prior to the effect of the service.
- Services not specified under the contract such as medical care and housekeeping tasks are not provided.
- Part-time caregiver services are available through NJ Sanhoo only. Simple house keeping services can be provided if requested in prior to NJ Sanhoo.
- If the caregiver is providing unsatisfactory services according to the contract and under reasonable expectations, a different caregiver may be substituted ONLY ONCE within 5 DAYS of the beginning of the service.
- During the duration of the service, under the permission of the client, NJ Sanhoo will provide assistant caregivers in case of emergencies and unforeseen events.
- NJ Sanhoo is not responsible if due to natural disasters or severe weather conditions the caregiver is unable to provide the service.
2) Service Reservation
Client may make reservations via telephone, fax, Internet or by visiting the office. After making the reservation, client must provide payment with the name of the client to NJ Sanhoo Once the payment is received and confirmed, the reservation will be processed.
After the confirmation of the payment, NJ Sanhoo will send a notice via telephone, mail or email informing client that the payment was received and reservation has been processed.
Once the payment is approved and there are no raised issues in regards to the contract, NJ Sanhoo acknowledges that client have agreed to the terms of the contract.
3) Use of Service Agreement
Following conditions will make this contract void:
Missing information in the client form
If client write someone else’s information
Not providing acceptable payment after making a reservation
Providing false information on the forms
Request of further services without advance notice following the completion of the service
Specific services or duties not request in advance will not be provided
Once the payment is checked, the reservation is completed. If cancellation of the serviced is requested immediately, NJ Sanhoo will provide a full refund.
4) Change of Service
Requests for modified services due to changes in due date or surgery date must be made AT LEAST 7 DAYS IN ADVANCE.
5) Reservation Cancellation and Refund
- Cancellation may be requested under special circumstances.
- Cancellation must be requested via the same method utilized to reserve the service (telephone, fax, email, internet or visitation).
- Cancellation must be requested at least 1 day in advance before the day of interview with NJ Sanhoo If cancellation request is made on the day of the interview, 50% of the deposit fee will be refunded.
- Cancellations requested on the day of the interview will incur one-day service charge of $145 deducted from the payment.
- If the service cannot be provided and is cancelled by NJ Sanhoo, full refund will be issued to the client.
6) Payment Authorization
NJ Sanhoo will receive a standard service fee.
If the service is provided during holidays or etc., additional fee of $30 will be charged.Deposit fee must be received and approved on the day of reservation. Payment for the remaining balance may be paid on the day of the interview. Grocery-shopping fee is additional $50.
NJ Sanhoo reserves the right to deny additional requested services (i.e. accompaniment visiting the hospital). Additional $30 fee will be charged for additional services approved.
Services requested in addition to specified working hours will be charged $24 per additional hour.
7) Length of Service and Extensions
Service duration must be minimum 2 weeks. NJ Sanhoo services are provided in weekly terms not daily.
Any requests for changes or extension of the duration of the service must be submitted for approval to NJ Sanhoo.
Requests for the extension of the service must be submitted AT LEAST 1 WEEK IN ADVANCE from the last day of the original service expiration date. If NJ Sanhoo is able to ap prove and extend the service, additional service charges will be applied. NJ Sanhoo may not be able to approve the request for service extension depending on the schedule and availabilities.
8) Service Cancellation and Refunds
- Client may cancel the service if NJ Sanhoo fails to provide services agreed under the contract.
- NJ Sanhoo reserves the right to terminate the contract if NJ Sanhoo or NJ Sanhoo caregiver is requested services not agreed under the contract or agreed upon (i.e. returning grocery items). SANHOO NYNJ LLC may also terminate the service if NJ Sanhoo caregiver is requested services by additional members of the family other than the mother, treated as a housekeeper or is treated in a disrespectful manner.
- If NJ Sanhoo caregiver is providing unsatisfactory services according to the contract and under reasonable expectations, a different caregiver may be substituted ONLY ONCE within 5 DAYS of the beginning of the service. If cancellation is requested after 5 days, NJ Sanhoo is not responsible for a full refund.
9) Obligations and Working hours of the Caregiver
- Work hours are weekdays from 9 am to 6 pm, if needed, service on saturday ends at 2 pm. Sundays are off.
- Live-in caregiver working hours are weekdays from 9 am to 9 pm and Saturdays 9 am to 2 pm. Further service hours may be requested with additional service charges.
- Live-in caregiver completion time (last day of the contract) will be weekdays 6 pm and Saturdays 2 pm.
- Caregivers are entitled to 1-2 hours of break time daily to ensure best quality service.
- Holidays are not considered a working day. For services on holidays, requests must be submitted to NJ Sanhoo.
10) Property Damages and Medical Treatments
- Property Damages
i. Safeguard personal valuables to avoid any miscommunications between client and the caregiver.
ii. NJ Sanhoo does not take full responsibility for lost items but will cooperate to resolve the issue.
iii. If there are damages made due to fault of the caregiver, NJ Sanhoo does not take responsibility. - Medical Treatments
NJ Sanhoo caregivers are prohibited to provide medical treatments. NJ Sanhoo does not take any responsibility for medical treatments during the duration of the contract. If medical treatment is or needs to be provided, NJ Sanhoo does not take any legal responsibilities. Contact NJ Sanhoo if NJ Sanhoo caregiver is providing any medical treatment.
11) Client Agreement
Client must actively participate in all activities specified under the contract.
Client is under full legal responsibility if a violation has occurred as a client.
If there are any issues, it is the duty of the client to report to the NJ Sanhoo immediately.
After the service has been completed, client may not privately ask the caregiver for additional services (babysitting, childcare, housekeeping, etc.) If client request additional services without agreement with NJ Sanhoo, NJ Sanhoo reserves the right to claim violation fees from the client.
Client must to adhere to NJ Sanhoo “Caregiver Courtesy Guideline.”
Personal Damages
i. During the duration of the service, client must agree and follow Section 10 of the contract.
ii. If the caregiver is injured during service, the client is partially responsible for the medical bills.
